hey there gorgies!!! On quite a few of my pages I put part of the photo outside the frame (mainly as the photo is too big for the frame :giggle:) and guaranteed I will get messages asking how I did it.
Here's a couple of examples (linked):

So ............... I will show you how - and you will kick yourselves as it is so easy really!

Step-by-Step:

1. So you have your photo underneath the frame. Make sure some of it (like an arm or a head) is outside or under the frame.
2. Copy the photo (control/command J). NB: before you do this make sure the photo is exactly the colour/sharpness/size you like as there is no going back if you want to change it later!!
3. Move this photo over the frame (so to the layer above the frame). *SAVE*
4. Using the eraser (E on your keyboard), simply erase the background around the arm or head or whatever, so you are left with the extracted body part over the frame.
5. Now you'll need to add shadow to bring it to life. Add drop shadow. Don't worry too much about the shade or the blur - but you can change the colour from black to whatever tickles your fancy. Don't worry if the shadow is in other parts like the middle or something.
6. Put the shadow on a new layer: right click on FX / drop shadow layer and click on 'create layer' Voila new layer created!
7. Click on this new (drop shadow) layer. *SAVE*
8. Now you'll need to use a bit of common sense here. Firstly you'll need to delete all shadow over the main photo upto the inner edge of the frame. You can use the eraser tool for this or marching ants - whatever works best for you.
9. SO your page should now have an arm (let's say) over the frame with shadow underneath. But you'll probably have to tweak that shadow. Remember when I said not to worry about the shade or the blur - now this is where your common sense kicks in - coz PS won't do this for you. Just change the opacity for the shade and the blur using the gaussian blur filter in the filter palette. Remember this is a seperate layer now so you can move it around or warp that shadow. Just make sure that none of the shadow is over the original photo and that you have shadow under the whole "arm". *SAVE*
10. Sit back and be proud! You just made an "out of the frame" page!!! WOOOOHOOOO!!!

You can use this method on a few other things too - like curled up corners of paper or in part of your cluster when one part should be over another.

This is the method that works best for me but you could erase the unwanted part of the photo by using the magic wand tool or the lasso/magic lasso tool. But I like the control of the eraser (plus I have a pen tablet which helps no end!!)
